- To respond to questions and appointment requests.
- To help coordinate scheduling and provide requested service information.
- To maintain and improve the functionality of our website and scheduling experience.
Appointment requests
Submitting a request does not confirm an appointment. Our team may contact you by the method you select about that request. If you select text messaging, message and data rates may apply and you may reply STOP to opt out.
Please do not submit sensitive information
Do not send symptoms, diagnoses, medical history, insurance identification numbers, payment information, or other sensitive health information through advertising or general website contact forms. Secure intake paperwork is provided separately when appropriate after scheduling.
Service providers and website data
Website, advertising, analytics, and scheduling providers may process information needed to deliver the functionality you request under their own terms and privacy practices. Our website may collect aggregate usage and cookie data. We do not intentionally include form answers, health details, or insurance details in website analytics events.
Clinical privacy
